Eldercare Planning Services

We help families find solutions for the care of their elderly loved ones. Helping seniors maintain quality of life takes time and specialized skills. Solutions for Seniors helps families care for aging seniors when they can’t be there themselves or don’t have the time. We provide services that assist long–distance caregivers who need a local expert to oversee their loved one’s care, and we notify them of changes or problems. For those who are no longer able to receive care at home, we can help ease the transition when a move to an assisted care or residential care facility becomes necessary.

OUR APPROACH

  1. Understand your situation
    An initial consultation allows us to listen to your concerns either in person or over the phone. We can answer your questions about eldercare planning and see how our services can benefit you and your loved one. This initial meeting also gives us the opportunity to acquire some background information about the aging senior and your family.

  2. Assess your situation

    In-home assessment: We offer a comprehensive evaluation, preferably done in the senior’s home, which includes evaluating how your aging loved one gets through the day. We also look at their quality of life including functional abilities, psychological and social functioning, medical history, financial resources, and environment.

    Short consultation: This option is ideal for long-distance caregivers or for those families who have a good understanding of their aging loved one’s situation. The short consultation consists of a 1–hour meeting or phone call in which we target service–care issues. The consultation identifies issues, assesses present conditions, and offers solutions.

  3. Create a personalized service-care plan
    From our assessment we develop a written service-care plan to address present issues and improve the quality of life and safety for your loved one. The service-care plan includes resources and services selected to meet your loved one’s personal preferences and interests. We encourage you and your family to review the plan and provide feedback on our recommendations.

  4. Implement and manage service-care plan
    Clients may choose to retain Solutions for Seniors to implement the service-care plan. We can help you with decision-making, care research, and service options as needed. We work with you and your family to establish how much care the family or friends want to provide and how much other health and service providers will be needed to assist. Implementation and management of the service–care plan may include:

    • Assessing housing options and assisting with relocation
    • Keeping track of medical appointments and, where possible, participating and advocating on behalf of the aging senior
    • Assisting with outings and arranging other activities that enhance the quality of the aging senior’s life such as meal planning, housekeeping, grocery or personal shopping, and cultural or social activities
    • Securing qualified private home care
 
If families wish, we monitor the aging senior’s care regularly or as needed. This process may include phone calls to the aging senior and the family, home visits, and communication with inter-agency professionals and other family members. We evaluate the success of the arrangements and adjust the service-care plan as the aging senior’s needs change.
  1. Keep in touch
    We provide a monthly report to each client detailing the services, the date and time of each service, and an overview of results. Our monthly reports are the most effective way to communicate with family members and caregivers the goals and outcomes of each action taken on the aging senior’s behalf.
